High Standard social workers play a big role in client care. Their primary role is to help clients and their families understand an illness, come to terms with the emotions of a diagnosis, and provide counseling on a variety of decisions. Social workers can be a family’s biggest ally, problem solver, and an invaluable resource. Our goal is to improve or maintain the emotional, physical, functional, and social health status of the client.
Interventions provided by the Medical Social Worker include:
Psycho-social assessments
Financial-environmental assessment
Counseling
Short-term therapy
Client, family and community education
Community resource planning
All High Standard social workers function in accordance with the High Standard Home Care code of conduct and the Standards for the Practice of Social Workers, including its accountability procedures.
